Mini Mandalas Yoga — Parent/Guardian Consent & Liability Waiver

In exchange for permission for my child to participate in Mini Mandalas yoga classes, I, the undersigned, hereby agree to the following terms:

Release of Liability

I, on my own behalf and as the parent/legal guardian of the participating minor child, fully release, discharge, and hold harmless Lisha Kritzinger, Mini Mandalas, and its directors, employees, landlords, lessees, sponsors, and franchisees (collectively referred to as the “Released Parties”) from any and all liability for personal injury, illness, or damage to property (whether mine, my child’s, or another person’s) arising out of or in connection with participation in Mini Mandalas yoga classes or related activities.

Health Acknowledgement

I confirm that I have been advised to consult with my child’s physician regarding any current or previous injuries, illnesses, medical conditions, or medications that may affect my child’s ability to safely participate in yoga sessions. I acknowledge that I have taken appropriate steps to do so.

Emergency Medical Treatment

In the event that my child becomes ill or injured during a Mini Mandalas session, I authorise the Released Parties to arrange emergency medical treatment as they, in their sole judgement, deem necessary to preserve the health and well-being of my child. I further release and hold harmless the Released Parties and any medical personnel involved from any liability arising out of such care or treatment.

Booking & Payment Policy

• We kindly ask that all bookings be made at least 1 hour before the class begins to allow for preparation and to ensure your child’s spot is secured.
• Payment is due no later than the day of the class session.
• Sessions may be cancelled by the parent/guardian, but no refunds will be issued, as venue hire and administration costs remain incurred.
• Exceptions may be considered at the discretion of Mini Mandalas in cases of emergency or illness.
